Licensed Practical Nurse
The State of Connecticut Department of Mental Health and Addiction Services (DMHAS) is currently seeking a dedicated and compassionate Licensed Practical Nurse to join the Western Connecticut Mental Health Network (WCMHN) within Behavioral Health Home (BHH) services in Danbury, CT. This position plays an important role in supporting individuals in recovery through integrated, person-centered care.

DISCOVER THE OPPORTUNITY TO:
- Provide nursing care for the Behavioral Health Home initiative within the LPN scope of practice
- Administer medications in accordance with nursing standards and DMHAS policies
- Support individuals in recovery through direct care and integrated services
- Document client contact and treatment to meet Joint Commission and agency standards
- Implement recovery plan interventions and maintain a safe therapeutic environment
- Co-lead psychoeducation groups and classes
- Collaborate with behavioral health and medical staff to ensure coordinated, holistic care
- Assist individuals in accessing services including health screenings, primary care, housing, transportation, and substance use treatment
- Deliver BHH services including care management, care coordination, health promotion, transitional care, individual and family supports, and community resource referrals

The network operates in Waterbury, Danbury, and Torrington, with over 200 dedicated staff offering comprehensive services for individuals with long-term mental illnesses.
