Clinician - Fulltime (Fairfield)

POSITION SUMMARY

This position is responsible for managing all clinical aspects of YCCC programs including training all staff, monitoring, and advising on treatment plans and counseling, review of client charts, and clinical oversight of agency programs. Clinician has responsibility for overseeing the services related to client treatment and medical records documentation and collaborating with staff to address all aspects of the clinical program.

*This position is for Tuesday-Saturday or Sunday-Thursday*

Hiring is contingent upon a full background screening in accordance with California licensing regulations; DOJ/FBI clearance, pre-employment physical and TB clearance. DMV printout and Basic First Aid certification is required prior to employment.

SUPERVISION/REPORTING RELATIONSHIPS

This position reports to the Program Director.

MINIMUM POSITION REQUIREMENTS

  • Education: Master’s degree in counseling, psychology, or social work and a valid license active for 2 years for clinical practice in California. Will consider those that are working toward licensing hours.
  • Experience: Two years of in a mental health program or clinical program preferred.
  • Knowledge: Understands tenants of psychosocial rehabilitation, counseling theory and ability to interpret regulatory requirements
  • Special Skills: Demonstrated ability to work as a team player. Excellent communication skills both oral and written.
  • Physical Requirements: Ability to lift 20 pounds.

1. Training and Knowledge Base

  • Awareness of current mental health programs including, Community Mental Health Specialty Mental Health Services / Mental Health Plan; Assertive Community Treatment (ACT); Assisted Outpatient Treatment (AOT)

  • California Association of Social Rehabilitation Agencies (CASRA) social rehabilitation model of care
  • Welfare and Institutions (W&I) Title Nine, Title 22, Medi-cal Law.

2. Direct Services

  • Addresses referrals from providers including referrals
  • Participates in daily huddle to assess needs and coordinate services
  • Provide therapeutic services to clients and their families
  • Work in collaboration with multidisciplinary team members including external partners
  • Completes mental health assessments and treatment planning and implementation
  • Demonstrates cultural competency and commitment to patients
  • Provides crisis intervention

3. Milieu Management/Treatment Environment

  • Provide information (verbally and in Communications Log) during shift transition
  • Support team members and utilizes a team approach when a crisis occurs
  • Be aware of and eliminate stressors in the milieu
  • Prepare and facilitate groups as assigned
  • Manage milieu to ensure clients feel safe and secure
  • Ensure that milieu is clean, safe and in compliance with Medi-Cal and Community Care Licensing requirements

4. Documentation

  • All charts, program documentation, MARs, incident reports, and other forms/reports are completed according to the following criteria:

o Timely

o Objectively (behavioral observations are recorded accurately)

o Thoroughly

o Legibly

  • Correctly follows documentation procedures
  • Treatment plan goals are developed in tandem with the client and are time limited, observable, measurable, and achievable
  • Progress Notes are to be completed during shift

5. Quality Improvement

  • In collaboration with Quality Improvement Director, collaborate and consult regularly regarding strategic plans
  • In collaboration with Quality Improvement Director, monthly agency management; QIC committee meetings
  • Attend and participate at county clinical meetings, including SUR and CIT
