Bilingual Clinical Case Manager

Clinicians need to be licensed in the state in which they will be practicing.

A license or QMHP certificate to practice in the state of Virginia is required

We are looking for Qualified Mental Health Professionals (QMHPs), Licensed Professional Counselors (LPCs), Licensed Clinical Psychologists (LCPs), or Licensed Clinical Social Workers (LCSWs) in Virginia with Case Management experience working with youth mental health and/or substance abuse issues and their families.

We are seeking for 3 Bilingual (Spanish/English) Clinical Case Managers who are proficient in both languages and hold a QMHP certificate or a professional license to provide services in Arlington Virginia. We will be working with youth and families under high stress, including those with acute mental health, addiction, and other challenges. Our direct clients will be teenagers referred to by county schools, court services, and others.

Minimum Qualifications:

  • A LCP, LCSW, LPC license or QMHP certificate to practice in the state of Virginia is required
  • Bachelor's degree in social work, psychology, or a related field.
  • Fluency in both English and a Spanish language, with strong verbal and written communication skills.
  • Experience in case management or a related field, preferably in a clinical or healthcare setting.
  • At least one year of clinical experience with children, adolescents and or families in addiction counseling, trauma therapy, family therapy, child school counseling, couples therapy, and career counseling.

Preferred Qualifications:

  • Master's degree in a related field, such as psychology, social work, counseling, substance abuse, or family therapy, is preferred.
  • Certification in case management or a related professional credential.
  • Experience working with diverse populations and understanding cultural competency.

Responsibilities:

  • Assess needs of participating in youth and their families and develop individualized service plans (ISP) with specific goals for both. The ISP must utilize accepted placement criteria and be completed within 30 calendar days of service initiation.
  • Connect participants to relevant programs and resources based on the identified needs.
  • Make necessary contacts with the participants’ significant others (with authorized releases) to support ISP implementation, community adjustment, health, and well-being. Link the participants with community supports most likely to promote their habilitative or rehabilitative recovery and life goals as developed in the ISP.
  • Conduct follow-up assessments to evaluate progress and address ongoing needs after initial service delivery.
  • Ensure that services are accessible to all participants, regardless of their insurance or citizenship status.
  • Monitor the quality-of-service delivery through regular and timely contact with participants and external providers. Conduct virtual, in-person, and home visits to assess the quality of care and participant satisfaction.
  • Provide follow-up education, instruction, and counseling to guide youth and families in building supportive relationships and achieving the goals outlined in the ISP.
  • Advocate for participants based on their changing needs and adjust the ISP accordingly.
  • Assist participants in planning and preparing for life transitions, ensuring ongoing support.
  • Follow up with participant’s health status, including medical conditions, medications, and potential side effects.
  • Assist participants in accessing primary care and other medical services as needed.
  • Coordinate and monitor services provided to clients, ensuring they receive appropriate support and resources.
  • Facilitate communication between clients, healthcare providers, and community resources, utilizing bilingual skills as needed.
  • Document case progress and maintain accurate records in compliance with organizational policies and regulations.

Skills:

The required skills for this role include strong assessment and analytical abilities, which are essential for evaluating client needs and developing effective care plans. Bilingual communication skills are vital, as they enable the Case Manager to connect with clients from various backgrounds and ensure they fully understand their options. Organizational skills are necessary for coordinating multiple services and maintaining accurate documentation. Empathy and interpersonal skills are crucial for building trust and rapport with clients, allowing for more effective advocacy and support. Preferred skills, such as advanced knowledge of community resources and cultural competency, enhance the Case Manager's ability to provide holistic care and address the unique challenges faced by clients.

 

Working Conditions/Physical Effort:

Work is performed mostly in an office setting. Some travel, public speaking, evening, and weekend work are requirements of the position. Regular computer work is also required. While performing the duties of this job, the employee is required to stand; walk; sit; finger, handle, or feel objects, or controls; reach; climb stairs; balance; stoop, kneel, or crouch; talk and hear. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
